Composition

Nepado Sterile Ophthalmic Suspension: Each mL contains Nepafenac INN 3 mg.
Preservative: Sodium Perborate 0.01%

Pharmacology

Nepafenac is a non steroidal anti-inflammatory prodrug (NSAIDs) for topical administration to the eyes. After instillation in the eye, it penetrates the cornea and is converted by ocular tissue hydrolases to amfenac, a nonsteroidal anti-inflammatory drug. Amfenac is thought to inhibit the action of cyclooxygenase, an enzyme required for prostaglandin synthesis.

Indications

Pain and inflammation associated with cataract surgery.

Dosage and Administration

Shake well before use.

For post-operative pain & inflammation: Instill 1 drop once daily 1 day prior to cataract surgery and continued on the day of surgery and through the first 2 weeks of the post-operative period. An additional drop should be administered 30 to 120 minutes prior to surgery or as advised by the registered physician.

Contraindications

Contraindicated in patients with previously demonstrated hypersensitivity to any of the ingredients in the formulation or to other NSAIDs.

Side Effects

The most frequently reported ocular adverse reactions following cataract surgery were:

• Capsular opacity
• Decreased visual acuity
• Foreign body sensation
• Increased intraocular pressure
• Sticky sensation

Warnings & Precautions

• Nepafenac may increase bleeding time.
• Caution should be exercised in patients with known bleeding tendencies.
• Nepafenac may slow or delay healing.
• Continued use may result in epithelial breakdown, corneal thinning, corneal erosion, corneal ulceration or corneal perforation.

Special Guideline

• If more than one topical ophthalmic product is to be used, other eye drops should be instilled at least 5–10 minutes apart.
• Use solution first, then suspension.
• If using eye drops and eye ointment, use drops first.
• Don’t share eye drops with another person.

Use in Pregnancy & Lactation

Pregnancy: Use only if potential benefit justifies the risk.
Lactation: Caution should be exercised when administered to nursing mothers.

Use in Children & Adolescents

Safety and effectiveness in pediatric patients below 10 years of age have not been established.

Drug Interactions

Concomitant use of topical NSAIDs and topical corticosteroids may increase healing complications.

Overdose

If accidentally overdosed, flush eyes with warm tap water.

Storage

Store below 30°C, protect from light and keep in a dry place.
Do not use longer than 30 days after first opening.
Keep out of reach of children.
